In line with the implementation of the National COVID-19 Vaccination Program,  the UP Open University  is encouraging UPOU students to coordinate with their respective LGU for the registration and scheduling of vaccination.

May we also request you to report your vaccination details from time to time on Self-Reporting of Covid-19 Vaccination for UPOU Students.  With this monitoring form, UPOU will provide updates to CHEd on the requested data on Student Vaccination Report.
